Social Services
Adult Services’ Case Management Unit provides
coordinated social services for seniors who are facing physical, mental, and/or
financial crisis. After going through a streamlined intake process, which
allows for gathering pertinent information in one sitting, each client is
assigned to one case manager who maintains primary contact with the
senior and is responsible for assessing needs, coordinating services, and
monitoring results. The case management team assists over 120 people
annually.
