Hartley House is a historic settlement house serving the Hell’s Kitchen neighborhood in New York City since 1897. Programs include after school and summer day camp programs for children, support services for neighborhood seniors.
The Home Outreach Program for Elders (HOPE) is looking for an energetic, responsible, and conscientious Social Worker to provide a full range of comprehensive case management activities in the Chelsea HOPE program. Under the supervision of the Director of HOPE, the Social Worker will further advance the ability of HOPE to meet the needs of seniors in providing direct services including bill pay assistance, in-home assessments, health and entitlement referrals, caregiver support, crisis intervention and support, and client advocacy. S/he will serve as resource to the Hudson Guild community in aging matters. Our approach is holistic and highly flexible so that we are able to meet the client at the level of care and need they present throughout service.
MSW is required. General knowledge of public benefits and entitlements. Experience with an aging population
Typical office environment may require occasional travel between sites.
Position requires frequent communication with participants, management, and staff. Must be able to remain in a stationary position, as well as the ability to operate a computer and other office machinery, such as phone and copy machine. Occasionally lift up to 20 lbs.
